1 – How to request the scheduling of examination sessions:

2 – Form certifying that an examination has been taken (upon request by a student)

3 – Forms for amending or cancelling digitally signed examination records:

Examination record amendment form

• incorrect grade
• incorrect date
• incorrect examination questions

Examination record cancellation form

• examination recorded under the wrong student ID number
• examination recorded under the wrong course code
• examination recorded for a student with irregular enrolment status
